How do I file a complaint with corporate on the insurance department

Customer service there is insufficient. I didn't receive any assistance at all. I asked to speak with a supervisor. The rep returned and said that the supervisor said that she would "try" to give me a call back. What do we do when we also get subpar service from their supervision? Why is their not a contact number for corporate? I'm extremely upset!
