why am I seeing 2 $0 pending Visa transactions? 1 merchant name is "Visa Provisioning Service". other is "None".

why am I seeing 2 $0 pending Visa transactions? 

I've spoken to 3 USAA reps about this and also tweeted USAA trying to get a satisfactory answer but without any luck.

there are 2 pending transactions on one of my USAA Visa cards.  Both have $0 amout.

1 merchant is "Visa Provisioning Service".  the other merchant name is "None".

1 USAA rep suggested that I could report these as fraudulent but how do I know whether they're fraudulen if USAA can't tell me the merchant.
